Below are the forms you’ll need to order supplies and services for the show.

A 10' x 10' BOOTH comes with ONE(1) 8' table, ONE(1) chair, ONE(1) wastebasket, and THREE (3) Badges.

An ARTISAN TABLE comes with ONE(1) 8' table, ONE(1) chair, ONE(1) wastebasket, and THREE (3) Badges.

BADGES - Each booth/table comes with THREE (3) badges.

PA Tax ID Number information
All vendors must have a PA Tax license to sell in the state of Pennsylvania, you must collect state sales tax 6% and remit it to the state.

A REQUIRED Township Merchant License will be given out during check-in and must be clearly displayed

THE EXHIBITOR KIT IS NOT AVAILABLE AT THIS TIME.
THE KIT WILL BE POSTED LATE 2009/EARLY 2010.

You MUST include this form if you are ordering extra tables, crossbars, uprights, carpet, or shipping anything to the decorator.

Shipping:
NO PACKAGES CAN BE SHIPPED TO THE HOTEL.
Any and all boxes will be turned away.
